When we call you back, tell us where the patient lives, and we’ll confirm whether we can provide hospice care there.
We accept Medicare Part A, Medi-Cal (Medicaid), major HMO insurance plans and private pay. When we call you back, tell us the patient’s plan, and we’ll verify coverage before care begins.
How a referral works
Step 1
You leave a request
Leave a call-back request and choose “For my patient.”
Your request is with our team.
Step 2
We call you back
We take the referral and ask how to reach you.
Your referral is with our team.
Step 3
Eligibility review
Our team and the hospice physician review the referral and the patient’s coverage, and check whether we can provide care where the patient lives. A hospice nurse visits the patient and assesses their needs.
The team can then confirm whether the patient is eligible.
Step 4
Care begins
Care begins only if the patient is eligible and chooses hospice care.
From then on, we keep you informed.
Working with you and the patient’s doctor
Physicians, hospitals, skilled nursing facilities, assisted living communities and home health agencies can refer patients.
The patient can keep their own doctor as the attending physician. We work with that doctor on the care plan and keep them up to date.
Questions about a referral?
