Our care team
A whole team cares for your loved one. Here’s what each person does and how visits work.

What each person on the team does
Hospice physician
The hospice physician oversees the medical care plan. Together with the patient’s own doctor, if there is one, they confirm whether the patient qualifies for hospice care.
Medical director
Our medical director leads the hospice team in planning care, confirms the attending doctor’s prognosis and advises the attending doctor.
Hospice nurse
Our skilled nurses are registered nurses (RN) and licensed vocational nurses (LVN). A hospice nurse visits to ease pain, shortness of breath and other symptoms. They explain each medication and keep the hospice physician up to date. The registered nurse coordinates the care plan and teaches the family how to care for their loved one and use their medications.
Hospice aide
A hospice aide helps with bathing, dressing, grooming and other personal care that’s part of the care plan. Their visits help with your loved one’s daily comfort.
Social worker
A social worker helps your loved one and the family with worries, practical questions and paperwork. They can also point you to other help nearby.
Chaplain
A chaplain offers spiritual support that respects the patient’s beliefs, whatever they are. This support is there only if the patient and family want it.
How visits work
Every part of care follows one care plan. We make it with your loved one and the family and update it as needs change.
The plan shows who visits, what each medication is for and who to call with a question.
Not everyone on the team comes each time. How often each person visits depends on your loved one’s needs.
If this is a medical emergency, call 911.
